If you are already a Guardiant patient, engaged caregiver or family member and are interesting in our advanced care membership where we proactive help you monitor and managed your health conditions, we are here to help.
Ask about our advanced primary care membership option. We provide state of the art biometric devices to collect and monitor your health on-going. This information is reviewed by our clinical team and helps alert us of changes in your health conditions and proactively reach out to you.
The monthly health monitoring helps give you and/or your caregiver added support in managing your chronic condition, assist with specialty referrals, medications, mental health, social and community health needs, all for a flat monthly fee.
It is like having a doctor in your family with Guardiant here when, and where, you made need us.
