Guardiant Announces Partnership with Capital Blue Cross of Pennsylvania

Caring for a geographically dispersed member population can be challenging, especially when they are aging adults who may live some distance from their healthcare provider. In addition, for those of us who have been fortunate enough to work with both commercial and Medicare Advantage members, we can appreciate the difference in member needs as well as how important taking an integrated whole person care approach is to serving the needs of today’s members.

Guardiant Health engaged with the Capital Blue Cross health and wellness leadership team to help them refresh and execute their Medicare Advantage strategy. The team looked at the new strategy through the eyes of their Medicare Advantage members, to understand differences in member mindsets (e.g., chronic, newly diagnosed, on the go). Then, they examined the interdisciplinary care management models to find opportunities for near-term improvement and determine where multi-month transformational changes are required.  

The team identified and began implementing changes that mattered most to Medicare Advantage members pre- and post-admission, proactive assessments and access to services, as well as ongoing comprehensive care management required to help members with multiple chronic conditions more effectively manage their care, in partnership with their primary care and specialty providers.
